Taylor's Pointe, a skilled rehabilitation and long-term care facility located in Cincinnati, Ohio, is currently seeking an energetic and compassionate PTA or COTA to assist with daily therapy operations. This position will require the candidate to work five 8-hour shifts and maintain a flexible schedule. Quality resident care and overall job performance are a priority for this position. The ideal candidate will strive to excel at restorative programming procedures and philosophies. Experience as an STNA and able to be on call is preferred. 

Responsibilities include, but are not limited to:

• Ensuring the direction of the Restorative Care Plan is followed per the assigned resident(s) and carrying out interventions as written.
• Ensuring timely documentation of all relevant findings related to the Restorative Care Plan and directing resident care per facility policy.
• Ensuring the applicable Resident Assessment Instrument (RAI) process is followed in accordance to State and Federal Guidelines.
• Ensuring timely notification of resident related information, including changes in resident condition (example: weight loss, skin condition changes), to a licensed nurse and the Therapy Director for further direction.
• Successfully assisting assigned residents with Activities of Daily Living (ADL’s), including but not limited to feeding, bathing, dressing, transferring, ambulation, locomotion, personal hygiene, and toileting.
• Assisting the facility in maintaining the cleanliness of the therapy gym and therapy equipment.
• Following Residents’ Rights policies at all times.
• Performing other related duties as directed by Therapy Director and/or person of authority.
